If you are currently listed on a joint account and would like to remove yourself, you will need to relinquish your rights to the account and any capital credits associated with it.
In order to relinquish your rights to an account, the attached form must be completed and notarized. The original, notarized form must be returned to our office either in person or by mail for processing.
